Fiber on a plant based diet: what the doctors say
Fiber on a plant based diet: what Burkitt, McDougall, Barnard, Ornish and other doctors say, where they agree and differ, with sources.

Our view
Fiber is the part of plant carbohydrate that our gut cannot break down, so it travels through unabsorbed; in 2022 McDougall stressed that it works as tiny sugar chains, not as a broom that scratches the intestine1. Meat, dairy and eggs carry none of it, and neither do oil or sugar, a point McDougall, Campbell, Barnard and Rogers all make2345.
The modern case for fiber starts with the surgeon Denis Burkitt, who worked for years in Uganda and found that the chronic diseases filling Western wards were almost never seen there67. Peter Cleave had blamed sugar, but Burkitt reasoned that sugar only exists once refining has taken the fiber out, so the missing fiber might be the real culprit8. That idea sent him around the world weighing stools and timing how fast food passed through the gut9. By his count, scientific papers on fiber went from about ten a year in the late 1960s to more than five hundred a year a dozen years later10. In December 1971 he spoke at McDougall’s hospital in Grand Rapids, Michigan, while touring with a new book on dietary fiber, and McDougall still calls that talk the one that changed his life11.
We give most weight to the clinicians who have fed patients this way for decades, and they land on the same practical point: eat whole plant foods, which bring their fiber with them1221314. They part ways on how much the fiber itself explains, which kinds matter, how much is enough, and whether refining, blending or powders change the picture2151617.
Where they agree
Western eaters get far too little: Burkitt put the American average near 15 grams a day against over 100 for our ancestors and much of the developing world12, Gardner said in 2026 that Americans reach about half the advised amount18, and Rip Esselstyn called the shortfall the country’s leading nutritional gap in 202219. McDougall ranked the loss of fiber in refined foods just behind too much fat and protein among the harms of the Western diet, and ahead of salt and sugar20.
They agree the best sources are whole plants: Barnard put beans at the top, then vegetables, fruits and whole grains4, Burkitt praised peas and beans and asked for far more bread, potatoes and pasta2122, and Rogers named beans, berries, oats and lentils2324.
On how fiber helps, Burkitt measured Western stools at roughly 80 to 130 grams daily, versus 300 to 500 grams where bowel cancer and heart disease were rare, and a transit time of about three days against roughly 36 hours in rural Africa or India2526. Rogers, drawing on Burkitt, explains that fiber holds water in the stool so it passes without straining, and that gut bacteria turn fiber into butyrate, which nourishes the gut lining and calms inflammation2728. Kahleova in 2025 likewise pointed to the short chain fatty acids that gut bacteria make from fiber29. Ornish has said since 1999 that fiber slows the rise of blood sugar and fills people up before they overeat3031, and Barnard said in 2026 that fiber supplies almost no calories and even carries some out of the body unabsorbed3233. Jenkins adds that sticky fiber holds bile acids in the gut, so cholesterol leaves with the stool34.
Several link low fiber to bowel trouble: Burkitt said high fiber eating had nearly ended the need for surgery on hemorrhoids and diverticular disease35, Pritikin argued in 1982 that the low fiber diets then prescribed for colon diseases were what caused them36, and Rogers in 2026 tied a low fiber diet to constipation and to many chronic diseases beyond the gut37.
Where they differ
The doctors differ on whether fiber is the cause or a marker of a good diet, on which fibers count, on how much to eat, and on refined grains, blended food and fiber products2153817.
Fiber itself or the whole diet
McDougall credits Burkitt but thinks he looked too narrowly at fiber, since the Ugandans he studied ate a near vegan diet built on starch, and it was the whole pattern that protected them2. Burkitt himself, asked what to change, named more fiber as step one and much more starch as step two, saying starch never fattens anyone22. Gardner sees the threat in today’s protein craze, since chasing protein through meat or fortified processed foods crowds out fiber further18. Campbell ends his China report by noting that fiber comes only from plants, which in his view points again to a plant based diet3.
Which fiber counts
In 2026 Jenkins said that for cholesterol the sticky fibers of oats and barley do the work, while wheat or corn flakes do not15. Campbell, by contrast, measured 14 kinds of fiber in rural China and saw each one track with less colon and rectal cancer, with no kind standing out39. In a talk posted in 2018 he also stressed that fiber is many different substances, not a single one40. Burkitt’s own emphasis was cereal food, especially brown or homemade bread, plus the soluble fiber of peas and beans4121.
How much
Burkitt’s benchmark was the more than 100 grams a day of our ancestors12. In 2000 Novick wrote that public advice ran from 25 grams a day up to 35 to 45 grams or more, and that the center where he worked asked for at least 3516. Rip Esselstyn said in 2022 that his own daily intake typically runs 80 to 100 grams, with roughly 35 coming from breakfast42. Kahleova cited research where roughly 21 grams of daily fiber went with 63% less depression, with intakes below 10 grams among some of those studied43.
Plants or fiber products
Barnard said in 2026 that a fiber supplement would also carry some calories away, but that he would still rather people eat their fiber in plants13. Goldhamer said in 2025 that overdoing fiber is unlikely unless someone uses fiber supplements or processed products made to add it17. Fiber cereals have a long history here: McDougall recalls that Burkitt passed through Michigan in 1971 on his way to urge Kellogg to make more high fiber cereal44. Rip Esselstyn’s 2022 fiber show was built around a PLANTSTRONG cereal sold by his company, and he described all of the brand’s whole food products as high in fiber45.
Whole, juiced or blended
Esselstyn’s FAQ rejects juice because fruit sugar without its fiber is absorbed too fast, and rejects smoothies because pureed food skips chewing1446. Ornish, writing in 2014 for people with type 2 diabetes, likewise chose whole fruit over juice47. Rogers said in 2026 that blending breaks down much of the fiber, while granting that it helps some people eat more fruit and vegetables24. Goldhamer goes the other way for damaged guts: patients with conditions such as ulcerative colitis may need steamed starchy vegetables or blended salads while they relearn to handle vegetables17.
White or whole grains
Ornish said in 1999 that milling brown rice and whole wheat into white products strips away the fiber and bran30, and Burkitt pressed for brown bread41. Rogers eats white rice anyway: in 2026 he granted it has lost its fiber, but noted it gets only about 1% of its calories from fat and carries less arsenic than brown rice38.
Older views and the colon cancer debate
Campbell credits Burkitt, working with Alan Walker and Hugh Trowell in Africa, with the finding that high fiber diets go with less cancer of the large bowel48. Burkitt recalled that after a London lecture around 1972 the British milling industry told the press that fiber had no value in human nutrition, and that the same millers and bakers later invited him to speak49. In 1999 and again in 2000 large studies in the New England Journal of Medicine found no protection from fiber against colorectal cancer or polyps, and Novick answered that the intakes tested, about 25 grams a day at most, were too low to show a benefit5051. Campbell’s China data pointed the other way for every fiber type, though most of those links fell just short of statistical significance, and he judged Burkitt’s theory still intact5253. McDougall’s own first step in 1971 was modest: he swapped white bread for brown in his bacon, lettuce, egg and tomato sandwich54.
What each expert says
DBDenis Burkitt
Burkitt, a surgeon famous first for the lymphoma that carries his name, became known as the fiber man in the second half of his career55. After hospital surveys across Africa, he turned Peter Cleave’s sugar theory around and proposed that the fiber stripped out by refining was what Western diets lacked78. In an interview with McDougall, undated but posted in 2013, he set out the stool weights, transit times and intakes behind his hypothesis252612.
McDougall heard Burkitt lecture in December 1971 and dates his interest in the causes of disease from that day11. He explains fiber as indigestible sugar chains found only in plants, and ranks its loss among the main faults of refined Western food120. He also faults Burkitt for singling out fiber when the whole starch based diet deserved the credit2.
Campbell’s China research found fiber intake at least double the American level, perhaps triple, with colon and rectal cancer rates about half39. He measured 14 fiber types, found all of them pointing the same way, and concluded that Burkitt’s theory holds3953.
Barnard ranks beans as the richest fiber source and notes that cheese, meat, eggs and refined foods supply none4. In 2026 he said a high fiber vegan diet cuts a few hundred calories a day without effort, and that he prefers fiber from plants to fiber from a supplement3213.
For Ornish, fiber is what separates whole carbohydrates from refined ones: it slows absorption and keeps blood sugar and insulin from spiking, as he said in 199930. He has also said fiber makes people feel full before they eat too many calories, on television in 2012 and again in 20193156.
Goldhamer recommends low density, high fiber foods so people feel full well before they are stuffed57. He doubts that anyone eating real food overdoes fiber, and looks instead for a digestive problem when a high fiber diet causes pain17.
In a 2026 video Rogers called fiber far more important than people assume, crediting it with feeding the gut lining through butyrate5828. He builds on Burkitt’s account of how fiber eases the passage of stool, and stresses that only plant foods contain it275.
In 1999 and 2000 Novick defended fiber against studies that found no effect on colon cancer, arguing that their subjects ate too little of it to test the question5051. He reported a minimum of 35 grams a day as the target at the center where he worked16.
Rip Esselstyn said in 2022 that almost all Americans miss the recommended fiber intake, while he eats 80 to 100 grams a day himself1942. His company sells high fiber whole food products, including the cereal his fiber show was built around45.
Fiber was Jenkins’s first research field and led to his glycemic index work59. In 2006 he said the evidence pointed back to traditional plant foods60, and in 2026 he singled out the sticky fibers of oats and barley for lowering cholesterol15.
Gardner said in 2026 that fiber is what suffers when people chase protein, and that his advisory committee wanted more beans, peas and lentils for fiber and gut health1861.
Kahleova said in 2025 that the fiber packaged with complex carbohydrates feeds gut bacteria whose products help the brain and metabolism29. She linked even a modest fiber intake to much less depression43.
Pritikin told McDougall in 1982 that his diet was the best one for colon problems from diverticulosis to hemorrhoids, and that it cleared up constipation too36.
On a plant based diet fiber comes with the food, and these doctors point to whole beans, grains, vegetables and fruit rather than juice, refined grains or fiber powders41413. How much to aim for, and how to go about it with a sensitive gut, are points where they differ, so those are questions for your own doctor. This page is education, not medical advice.
