Whole grains on a plant based diet: what the doctors say
Whole grains, bread and flour on a plant based diet: what McDougall, Esselstyn, Barnard and Novick say about flour, white rice and gluten, with sources.

Our view
A grain kernel has three parts: the fibrous bran outside, the germ that would sprout, and the starchy endosperm in the middle, and refining strips away the first two1. Jeff Novick adds a second step to watch: refining removes bran and germ, while processing grinds the kernel, whole or not, into flour2.
John McDougall calls cereal grains the most important food people have ever eaten, and the doctors on this site build their meals on them, from Caldwell Esselstyn’s whole grain cereal, bread and pasta to Nathan Pritikin’s brown rice, corn tortillas and millet345. What worries them is the form most people eat: Novick said in 2016 that some 90 percent of American carbohydrate comes as white flour, sugar, white rice and white pasta, and Alan Goldhamer put refined carbohydrate at 86 percent in 202667.
We give most weight to the doctors who have fed patients this way for decades, and their common ground is plain: whole grains, eaten close to how they grow, belong on the plate, while refined flour and sugar products do not389. Where they part is on how much bread and flour a person can eat, whether white rice is acceptable, and whether gluten matters, and we take those as questions of degree101112.
Where they agree
All of these doctors put whole grains near the center of the diet: Esselstyn named cereal, bread, pasta, rolls and bagels made from them in 2018, and T. Colin Campbell said in 2017 that whole grains beat refined carbohydrates like white bread813. Pritikin built the diet he adopted after his diagnosis on whole grains with beans, peas, fruit, vegetables and potatoes14.
They also agree that refining changes the food: Dean Ornish said in 1999 that milling the bran and fiber out of whole wheat flour or brown rice makes a simple carbohydrate of a complex one15. Rip Esselstyn makes the same point, describing white bread, white pasta and white rice as mostly endosperm with the fiber, vitamins and minerals gone1. Hana Kahleova notes that a widely used plant food index scores whole grains as healthy and white bread and other processed grains as unhealthy16.
The population data they cite point the same way: Ornish pointed in 2008 to the Nurses’ Health Study, where women eating more whole grains had less heart disease17, and Novick wrote in 1999 of a study in Iowa women where daily whole grain foods went with about 15 percent lower mortality18. Walter Willett said on a 2020 panel that intact whole grains act very differently in the body from refined starches19.
Where they differ
The doctors disagree on flour and bread, on white rice against brown, and on gluten, while none of them argues against intact whole grains101120.
Intact grains or flour
Novick reserves the name whole grain for unground kernels like oats, barley, buckwheat groats and brown rice, at around 500 calories per pound cooked, against about 1,200 to 1,900 for breads, crackers and cereals made from whole grain flour10. In a 2016 talk he set a pound of oatmeal at 325 calories and a pound of whole wheat bread at 1,12021. McDougall, in a talk posted in 2021, called flour a processed food that is a problem for people who want to lose weight faster22, and in 2013 Novick reported that Esselstyn limits flour products only for patients whose weight will not come down23.
Chef AJ goes further: her weight loss program asks people to abstain from flour along with sugar, oil, alcohol and salt24. In 2020 she even treated rolled oats with caution, saying that once flattened they behave more like flour than the whole oat groats they come from25. Doug Lisle is more relaxed: in 2026 he said he eats whole grain bread, not white, finds white flour a bit more of a problem, and does not worry about the odd flour tortilla26. Denis Burkitt, interviewed by McDougall, wanted people to eat far more brown or homemade bread, as their ancestors once did27.
White rice and white bread
Peter Rogers eats white rice by choice: in 2024 he said brown rice holds more fat, spoils sooner and carries more arsenic in its outer layer11. In 2026 he granted that milling takes most of the fiber out of white rice while noting that fat supplies only about 1 percent of its calories, and cited the leanness of peoples who live on rice2829. Pritikin took the other side in 1982, saying he would advise no one to eat white rice, which lacks vitamins, minerals and fiber30.
Barnard sits between them: in 2023 he cited a Tufts study in which the fiber of brown rice both curbed appetite and carried some calories out unabsorbed31. He has also said white pasta is not what makes people fat, though whole grain spaghetti is better for its fiber32. McDougall used white bread to make his point in a December 2011 newsletter: in a 1979 Michigan State study, 16 overweight young men added 12 slices of bread a day, white for one group and high-fiber for the other, and both groups lost weight over eight weeks, the high-fiber group more33.
Gluten and wheat
McDougall wrote in a March 2013 newsletter that gluten-free diets are unhealthy for people who do not need them, which he put at all but about 1 percent of the population12. He noted that people with celiac disease must avoid gluten for life, and that overweight people with celiac disease often gain weight on gluten-free products3435. Goldhamer also puts celiac disease at about 1 percent, and in 2025 described a theory that gluten may set the immune system against the thyroid or joints in people with the same genes20. In a 2022 video McDougall denied that grains are inflammatory and said the research implicates animal foods36.
What each expert says
JMJohn McDougall
In a January 2008 newsletter McDougall wrote that cereal grains are 70 to 83 percent starch by calories and named them the leading food of humankind3. He answered the charges against grains by pointing out that more than 3 billion people live on grain-based diets without obesity, and cited genetic work showing extra copies of the starch-digesting amylase gene in humans compared with great apes3738. He still flags flour as a brake on faster weight loss22.
In a lecture posted in 2014 Esselstyn listed whole grains for cereal, bread and pasta first among the foods his patients eat4. Novick reported in 2013 that Esselstyn keeps intact whole grains on the menu even for weight loss23. For high triglycerides, his site’s FAQ advises cutting simple carbohydrates, white flour products among them39.
In a 2017 interview Campbell called whole grains the better choice and rated white bread and sugar poorly, and that most carbohydrate comes from vegetables rather than bread1340. In 2023 he said rice and potatoes are great and that brown rice or whole wheat probably beats the refined versions41.
In a 2022 Q&A Barnard recalled 1960s Japan, where rice came with nearly every meal and people stayed among the leanest nations until fast food and dairy came in42. In 2021 he said oatmeal and bran cereal help hold blood sugar steady43. He favors whole grain pasta and brown rice for their fiber without condemning the white versions3231.
Ornish draws a hard line between whole and refined grains, arguing in 1999 that stripping the bran makes a complex carbohydrate simple15. In an interview posted in 2011 he said he had eaten oatmeal that morning and that pasta is fine with other high-fiber foods if the grain is whole44.
Goldhamer counts minimally processed whole grains among the healthy foods, in a whole plant diet that leaves out added salt, oil and sugar9. In 2026 he said that grain advice is shaped by the way people actually eat grains, as breads, crackers and cookies full of sugar, salt and oil7. He has described celiac disease as affecting about 1 percent of people20.
Rogers prefers white rice to brown, citing fat content, shelf life and arsenic, and rinses his rice twice11. He scores oats well on fiber, and in 2026 said the only pasta he buys lists one ingredient45. In 2022 he called oatmeal a good starch, if slightly higher in fat than the others46.
Novick wrote in 1999 that whole grains supplied only about 1 percent of the energy Americans ate, against more than 20 percent from refined grains47. He reserves the term whole grain for intact kernels and treats whole grain flour products as calorie dense10. In 2016 he put the average American adult’s whole grain intake at under one serving daily6.
In a 2022 talk on fiber Rip Esselstyn explained the bran, germ and endosperm of a grain and said refined breads and pastas keep only the endosperm1. He wants grains eaten with their fiber still in them1.
Chef AJ’s program for food addiction rules out flour along with sugar, oil, alcohol and salt24. In 2020 she explained that oat groats need about an hour of cooking and steel-cut oats about 40 minutes, while rolled oats cook in about five and dissolve much like flour4825.
Willett said in 2020 that the quality of carbohydrate matters far more than its share of the diet, with intact whole grains standing apart from refined starches19. He added that much of the world now eats too much carbohydrate, more and more of it refined19.
In 1982 Pritikin described his program’s grain dishes as running from pasta, lasagna and pizza made its own way to brown rice, corn tortillas and millet5. He refused to recommend white rice to anyone, even as he cited a study where it covered the protein needs of young men30.
Kahleova explained in 2024 that the plant based diet index used in many studies puts whole grains with the healthy plant foods and white bread with the unhealthy ones16. In a 2022 talk she listed whole grains among the healthy foods that raise scores on the Alternative Healthy Eating Index49.
Lisle treats bread and tortillas as foods of middle calorie density that he eats without worry, choosing whole grain bread over white at home26. He finds a vegetable sandwich on whole grain bread satisfying enough that he does not overeat it50.
Burkitt told McDougall that people should eat far more cereal foods, especially brown or homemade bread, noting that ancestors ate about a pound or more a day while the English of his time ate under a quarter pound27.
We eat grains mostly whole and unground, like oats, corn, barley and brown rice, as these doctors advise, and treat flour products as the richer form102322. How much bread to eat, whether white rice is acceptable and whether to avoid gluten are the points where they differ, so those are questions for your own doctor.
