Calcium, dairy and bones: what the plant based doctors say
Calcium, dairy and bone health: what McDougall, Esselstyn, Campbell, Barnard and other doctors say about milk, greens and osteoporosis, with sources.
Updated . For education only, not medical advice.
Our view
Milk is sold as the food for strong bones, yet the doctors on this site see it otherwise: cows build their bones from the calcium in plants, and Barnard says people can do the same through green vegetables1. McDougall and Pritikin both recalled a World Health Organization search that turned up no cases of plain calcium deficiency23.
What the low-fat clinicians stress instead is calcium loss: McDougall, Pritikin, Campbell and Rogers all describe animal protein as an acid load that the body buffers with calcium taken from bone4567. Goldhamer, the Ornish program and Barnard’s PCRM add salt to that list, and Goldhamer also names coffee, soda and alcohol891011.
We give most weight to the doctors who have fed patients this way for decades with nothing to sell on the point, and they agree that dairy is not needed for bones: Caldwell Esselstyn’s FAQ calls calcium pills unnecessary and Goldhamer said in 2022 he would not usually recommend them1213. Walter Willett takes a milder line, saying in 2020 that a little dairy can fill a gap for people short of other sources14.
Where they agree
Plants supply enough calcium: Esselstyn’s FAQ points to greens, grains and legumes, Barnard to greens, beans and tofu, and Rip Esselstyn to leafy greens and beans121516. Willett, the one voice here who still allows some dairy, agreed in 2020 that healthy bones do not hinge on drinking a lot of it14.
Most of them also hold that more dairy does not mean stronger bones: Campbell said in 2017 that countries with the highest calcium intake have the most fractures, and Rip Esselstyn and McDougall made the same point about hip fractures and osteoporosis171618. Barnard’s PCRM cites a Harvard study following 78,000 women, where milk at least twice daily went with more hip fractures, not fewer19.
Bones depend on more than food, and several of them say so: Barnard and Rogers name vitamin D from the sun and exercise that loads the bones2021, Goldhamer adds weight bearing exercise and a check of vitamin D levels1322, and Burkitt thought exercise and the hormonal change after menopause count for more than calcium intake23.
Where they differ
The doctors differ on whether any dairy has a place, how much calcium people really need, what most weakens bones and which plant foods serve best2425262327.
Any dairy at all
Caldwell Esselstyn allows none: a lecture of his posted in 2014 lists milk, butter, cheese, yogurt and ice cream as foods that damage arteries, and his FAQ rejects even fat free milk because of its animal protein2428. Rip Esselstyn’s program also removes all dairy from the first week, and Barnard said in 2023 that people do better without dairy calcium2915. Others made room for some: Pritikin kept a little nonfat dairy in 1982 only so his diet would not put people off, though he called dairy unnatural for humans3031. Ornish listed nonfat dairy in his reversal diet in 2011, and his 2022 guidelines still allowed up to two servings a day as an option3233; by 2019 he called it an early concession and said his later studies had left it out34. Willett said in 2020 that cutting back to a single daily serving would already mean a lot, as long as sugary drinks do not take its place25.
How much calcium
McDougall said in 2013 that the World Health Organization set 400 mg a day while the US asked for 800, a gap he put down to the dairy industry26. Willett said in 2020 that people do need some calcium14. On pills, Esselstyn’s FAQ and Goldhamer both see no general need, and Burkitt doubted calcium tablets do much, noting that South Africans broke their hips about eleven times less often than Westerners while eating less calcium121335.
What weakens bones
For McDougall and Pritikin the main driver is protein: above roughly 16% of calories from it, both said, the urine carries off more calcium than the diet supplies, even with plenty of milk45. Campbell in 2014 and Rogers in 2025 traced this to the acid formed as animal protein breaks down, and McDougall called cheese the most acid-forming food of all6736. The Ornish program’s site frames osteoporosis as calcium intake against calcium loss, with meat, dairy and salt all raising the loss3710. Burkitt, by contrast, told McDougall he did not know the cause, and he leaned toward hormones after menopause and too little exercise23.
Which plant foods
Barnard ranks greens by how well their calcium is absorbed: broccoli, Brussels sprouts and kale rate high, spinach and chard low, and he said the body takes up about 32% of the calcium in milk but twice that, 64%, from Brussels sprouts27. Novick prefers calcium per calorie, citing in 2007 about 272 mg in 25 calories of watercress, and his site reprints a 2020 essay by John M. Friedman Jr. that puts greens ahead of skim milk once absorption is counted3839. Rip Esselstyn favors greens and beans, and his PLANTSTRONG brand also sells plant milks described as a source of calcium1640.
What each expert says
JMJohn McDougall
In a 2013 lecture McDougall said the World Health Organization found no cases of calcium deficiency and set a lower daily figure than the US, which he blamed on the dairy industry226. He sees excess protein as the cause of bone loss, with dairy, and cheese most of all, as the most acid-forming foods436.
Esselstyn allows no dairy of any kind, butter and ice cream included, seeing it as harmful to the arteries, and his FAQ rules out fat free milk too2428. The same FAQ says calcium pills are not needed because leafy greens, whole grains and legumes supply plenty12.
In 2014 Campbell explained that sulfur amino acids from animal protein turn to acid, which the body offsets with calcium drawn from bone, so he expects more fractures on diets rich in animal protein641. In 2017 he noted that the countries eating the most calcium break the most bones, a point he made as someone raised on a dairy farm1742.
Barnard said in 2024 that cows get calcium from plants and people can too, from greens whose calcium absorbs better than milk’s127. In 2023 he told a questioner that people do better without dairy calcium, getting it from greens, beans and tofu, with vitamin D and exercise that loads the bones1520. His PCRM cites more hip fractures among women drinking the most milk and warns that sodium raises calcium loss1911.
Ornish’s program long allowed nonfat dairy, in 2011 and still as an option in 2022 guidelines3233. In 2019 he called the cup of skim milk or yogurt a concession of the early years and said his later studies dropped it34. His program’s site describes meat, dairy and salt as raising the calcium lost in urine10.
Goldhamer said in 2022 that he generally would not advise calcium pills for bone health, pointing instead to whole plant foods rich in minerals and to exercise that bears weight13. He listed coffee, soda, alcohol and sulfur rich animal foods, dairy included, as pulling calcium from bone, and suggested checking vitamin D822. In 2025 he named excess salt as well9.
In 2021 Rogers labeled milk “liquid meat” because of its saturated fat, adding that osteoporosis rarely stems from too little calcium4344. In 2025 he argued that milk raises osteoporosis risk through the acid load of animal protein, and in 2022 he named sun and exercise as ways to keep bone mass721.
Novick judges calcium sources per calorie, and in 2007 he singled out watercress as one of the richest, while noting how little of it people eat38. In a 2016 talk he cited traditional Okinawans, who ate almost no animal foods, as having about 80% less osteoporosis45.
In a 2018 talk Rip Esselstyn said greens and beans give better absorbed calcium than dairy, and called cheese the biggest source of saturated fat in the diet1629. A 2022 post on his site claims animal foods lead the body to take calcium out of bone, and proposes drinking water or a plant milk instead46. His PLANTSTRONG brand sells plant milks marketed as a calcium source40.
On a 2020 panel Willett said that trials replacing dairy with nuts, legumes, soy and other plant proteins show better blood lipids, and long-term studies link the swap to less heart disease47. He said bones do not need much dairy, though a small amount can fill gaps, and that cutting back to a single daily serving would be real progress1425. He added that dairy and red meat supplied most of the saturated fat in the people his studies followed48.
In 1982 Pritikin called dairy unnatural for humans and kept only a little nonfat dairy in his program, noting how common lactose intolerance is3031. He saw no risk of calcium deficiency and blamed osteoporosis mainly on protein above about 16% of calories35.
Burkitt told McDougall that calcium was no reason to drink milk, since peoples who give it up after childhood rarely get osteoporosis23. He doubted calcium tablets help, citing far fewer hip fractures in South Africans who ate less calcium than Westerners35.
We get our calcium from plants, mostly greens such as kale, broccoli and bok choy, along with beans and tofu, as these doctors suggest151216. We leave out dairy, keep salt modest and stay active outdoors, since the doctors here tie bone health to calcium loss, sun and exercise as much as to intake131121. Whether you need a bone scan or a supplement is a question for your own doctor. This page is education, not medical advice.
