Cancer and a plant based diet: what the doctors say
Cancer and a plant based diet: what McDougall, Campbell, Ornish, Barnard and other doctors say and what their studies found, with sources.

Our view
The doctors on this site start from the same observation: the common Western cancers of the breast, prostate and colon were far rarer in places such as rural Japan, rural China and Kenya, where people ate mostly starches and vegetables123. From that, McDougall, Campbell, Esselstyn and Rogers conclude that a rich diet of meat, dairy and fat drives these cancers, while Burkitt, the surgeon who mapped cancers in Africa, put the share of American cancers due to diet at perhaps a third24567.
Most of this rests on comparisons between countries, on laboratory rats and mice, and on single cases, which can point to causes but cannot prove what a diet does for one patient8910. The clearest human trial cited by these doctors is Dean Ornish’s: 93 men with early prostate cancer, all of whom had declined standard treatment, were split at random, and after one year the men who changed diet and lifestyle saw their PSA fall while the comparison group’s rose11. That trial measured a blood marker and the effect of the men’s blood on tumor cells in a dish, not survival12.
So we give most weight to what these doctors agree on and to their own cautions: a low-fat diet of whole plant foods is the best case they make for lowering risk, and none of them offers diet as a replacement for cancer care131415. When it comes to treating a cancer already there, even the strongest believers framed their view as opinion or as not yet proven, Campbell in 2023 and Pritikin in 19821610.
Where they agree
They agree that cancer rates move with diet: Esselstyn described Japanese families whose breast cancer rose to American levels after migrating, and Ornish made the same point about prostate cancer in Japan and the United States117. McDougall and Pritikin both pointed to populations living on rice, vegetables and little fat that rarely got breast, colon or prostate cancer218.
Animal protein and meat draw the most blame: Campbell calls casein, the main protein in cow’s milk, a strong promoter of tumors in his own reading of the evidence, and Rogers and Rip Esselstyn repeat that view192021. Barnard in 2025 and Novick in 2007 both tied red and processed meat to colorectal cancer, and Rip Esselstyn has warned about the compounds formed when meat is charred222324.
Fiber and the colon are another shared point: Burkitt held colon cancer to be almost wholly a matter of diet, and Rip Esselstyn in 2022 called fiber a strong defense against it2526. Novick and Rip Esselstyn both class alcohol as a known cause of cancer2728.
Where they differ
They part ways on what diet can do once a cancer is found, on fasting, on how much of cancer diet explains, and on which pills or foods help, and some views carry their dates7293031.
Diet after a diagnosis
McDougall, who says a journal accepted his study of diet for breast cancer in 1982, the first of its kind, said in 2011 that research shows a diet change helps people live longer after common cancers, without giving study sizes in that video3233. Campbell said in 2023 that, from early data, such a diet may hold a cancer steady, calling this his opinion and the choice a personal one16. Burkitt, in an interview posted in 2013, doubted that diet changes much once a tumor has formed and said he knew of no figures on it7. Ornish and Barnard both stressed that the prostate trial does not show standard treatment to be unneeded1514.
Fasting
Longo described mice that fasted before very high doses of chemotherapy surviving while fed mice died, and his 2015 paper found that repeated cycles of a fasting-like diet cut tumors in middle-aged mice349. Goldhamer, who runs a fasting center, cited that animal work in 2025 and said human studies had begun35. In 2026 he said every lymphoma his center had seen seemed to respond to fasting, pointing to published case reports and series, though he said a clinical trial was still lacking29.
How much of cancer diet explains
Campbell, judging from the gaps between countries, estimated in 2023 that eating whole plant foods might prevent 80 to 90% of cancers or more, calling it his view30. Burkitt estimated perhaps a third of American cancers come from diet7. Nelson in 2022 said a low-fat plant based diet lowers risk but offers no guarantee, and faulted Joel Fuhrman for calling common cancers completely avoidable1336.
Sugar and keto
McDougall in 2017 rejected the low-carb claim that sugar feeds cancer, noting that starch eaters such as the Seventh-day Adventists of Loma Linda show lower rates of breast, colon and prostate cancer3738. Rogers in 2026 described mice on keto whose original breast tumors grew more slowly but spread more often to the lungs, and judged keto for cancer unwise3940.
Pills and single foods
Barnard credited cruciferous vegetables with a specific anticancer action in 2024, through sulforaphane41. Novick was more doubtful of single items: in 2008 he found no evidence that chocolate prevents cancer and called the case for tea far from settled, and in 2011 he rated the evidence for vitamin D against cancer as weak424331. Nelson, speculating in 2022, suggested supplements and especially DHA as one possible factor, citing studies that tie DHA to higher cancer risk44.
Older views
In 1982 Pritikin leaned on the National Cancer Institute, which he said had singled out dietary fat as the chief cause of these cancers45. Esselstyn put Kenya’s lower breast cancer rate at 20 times in a 2009 talk and at 30 to 40 times in a lecture posted in 2014461.
What each expert says
JMJohn McDougall
McDougall blames the rich Western diet, heavy in fat, cholesterol and chemicals, for breast, prostate and colon cancer, and points to Asians living on rice and vegetables as the contrast2. In 2011 he said diet also matters after a diagnosis33. In 2017 he called the idea that sugar feeds cancer a low-carb notion that starch-eating populations contradict3738.
Operating on breast cancer at the Cleveland Clinic, Esselstyn grew frustrated that each operation did nothing to protect the patient who came next, which turned him to nutrition46. He chose heart disease for his research because results come faster, hoping the same eating would also lower the common Western cancers47. His site’s FAQ, as we read it in 2026, credits plant based eating with a large drop in prostate cancer, and in a 2021 panel he cited Ornish’s finding that serum from plant based eaters slowed prostate cancer cells4849.
Campbell’s rat studies on liver cancer found that a diet with 20% casein promoted tumors while 5% casein, or soy or wheat protein at the higher level, did not, work he described in a talk posted in 20185051. His China study, run with the Chinese government in the 1980s, surveyed 65 counties and 130 villages; it compared regions and had no treated and untreated groups3. In 2023 he said nutrition, more than chemicals, decides whether cancer develops4.
Retelling Ornish’s trial in 2020, Barnard spoke of 84 men whose prostate cancer was progressing slowly: in the control group PSA climbed roughly 6%, while in the vegan group it went down, which he tied to lower IGF-152. He added that a diet change is no substitute for seeing a doctor14. In a cancer prevention talk posted in 2019, Barnard explained that the more fat a woman eats or carries, the more estrogen she makes, a hormone that can push breast cancer cells to grow53.
With urologists Peter Carroll and William Fair, Ornish randomly assigned 93 men whose prostate cancer was confirmed by biopsy and who had opted against treatment; after a year, serum from the men who changed cut the growth of cultured tumor cells by 70%, compared with 9% for the untreated control men1112. His program’s 2012 video points out that far more men die with prostate cancer than because of it54. By 2019 he was citing trials where untreated men with early disease had survival close to that of men treated with surgery or radiation55.
Goldhamer explains the fasting idea this way: normal cells adapt to low glucose and high ketones, while cancer cells do not and are left more exposed to treatment and the immune system56. Asked in 2026 about prostate cancer, he named diet, sleep, exercise and fasting as his first choices, citing Ornish’s research, and said he leaves medical management to the center’s physicians57.
The first item on Rogers’ 2022 cancer prevention checklist was to eat no animal foods at all, dairy, fish and eggs included5. He links animal protein to methionine, which cancer cells need, and to the growth signals mTOR and IGF-15859.
In 2007 Novick reported a large study in which the biggest eaters of red and processed meat had more colorectal and lung cancer23. The same year he wrote about the monks of Mount Athos, who ate mostly vegan, fasted often and worked daily, and whose prostate cancer rate was below one fourth of the global average60.
In a 2022 video on several known vegans who got cancer, Nelson said eating low-fat and plant based, with a healthy lifestyle, lowers the odds but guarantees nothing13. He said nobody can know the causes in those cases and speculated about supplements, DHA in particular44.
In a talk given around 2009, Rip Esselstyn blamed animal protein for promoting tumors and, during breast cancer awareness month, called eating plant based the cure the charity walks were looking for2161. In 2022 he tied slow movement of animal foods through the colon to colorectal cancer and named fiber as a defense26.
In 1982 Pritikin told McDougall his diet could do people with cancer no harm and could perhaps help, but conceded that he had only isolated cases and no proof10. He described one woman with a stomach cancer given months to live who improved on his diet and stayed free of symptoms for the two years he followed her, though he allowed that spontaneous remission happens62.
Asked in 2024 by a viewer facing a poor prognosis after cancer treatment, Lisle said he would commit fully to healthy living rather than feel doomed63. He noted that a five-year survival figure of 11% still means 11 people out of 100 alive64.
Burkitt’s first research was the jaw tumor in Uganda that carries his name6. He argued that catching and treating a cancer early does nothing to make it less common, since only cutting its causes can do that65. He saw colon cancer as a disease of diet almost wholly, and tied breast cancer mostly to fat25.
Longo’s explanation for his mouse results is that healthy cells protect themselves when food is short, while cancer cells cannot66. His 2014 study of 6,381 American adults tied a high protein intake between 50 and 65 to roughly a fourfold cancer death rate, a survey finding with no diet assigned67.
We eat the low-fat, whole plant food diet these doctors describe for lowering risk, and none of them presents it as a substitute for cancer care131415. If you have cancer or are being checked for it, make every decision about treatment, fasting, supplements and medicines with your own doctor and oncologist, and never stop or replace a treatment on your own.
