Saturated fat on a plant based diet: what the doctors say

Saturated fat on a plant based diet: what Esselstyn, McDougall, Barnard, Willett and other doctors say about animal foods, coconut and heart risk.

Updated . For education only, not medical advice.

Our view

Most saturated fat in a Western diet comes from animal foods: in Walter Willett’s cohorts it came mainly from dairy and meat, and Neal Barnard names dairy, with cheese at its head, as the largest single source12. A handful of plant foods are rich in it as well, above all coconut and palm oil, which Barnard and Rip Esselstyn’s site both single out34.

Nearly every doctor on this site treats it as a cause of artery disease: Caldwell Esselstyn long saw it as the main culprit, Peter Rogers traces the evidence back to Ancel Keys in the 1950s, and Willett says controlled feeding trials leave no doubt that it raises LDL cholesterol567. When low-carb writers and some meta-analyses argued it was harmless, Jeff Novick, Dean Ornish and Willett each replied that the studies behind that claim never lowered it far enough, or measured it against refined carbohydrate and trans fat rather than whole plant foods8910.

We give most weight to the low-fat clinicians, Esselstyn, John McDougall, Novick, Rogers and Jeff Nelson, who hold that trading saturated fat for plant oil is not the fix and that a diet of whole starches, vegetables and fruit keeps both total and saturated fat low111213. We also take on board the warning from McDougall and T. Colin Campbell that a nutrient label can hide the real point, which is the animal foods that carry it1415.

Where they agree

They agree on where it comes from: Barnard showed in 2014 that a day of two eggs, one bacon strip, a chicken thigh without its skin, some milk and a single-serving pizza already adds up to about 25 grams16. Rip Esselstyn called cheese probably the biggest source in the diet, and Rogers called milk liquid meat for its saturated fat in 2021, then gave fish’s saturated fat as one reason against eating it in 2022171819. David Jenkins said in 2026 that lard and beef tallow plainly push cholesterol up, and Denis Burkitt told McDougall that farmed meat holds several times the fat of wild meat, more of it saturated2021.

They also agree it harms more than the heart: in Chicago data Barnard presented, people taking in about 25 grams daily had more than triple the Alzheimer’s risk of people taking in about 1322. Ornish said in 2014 that red meat and saturated fat hurt the memory centers of the brain, and Rogers devoted a 2026 lecture to saturated fat as a cause of dementia2324.

On coconut and palm oil the agreement is close to complete: Barnard said in 2024 that both raise cholesterol, Rip Esselstyn’s site puts coconut oil at 92 percent saturated fat, and Esselstyn lists both among the oils he keeps out252627. Novick compared coconut oil with sugar in 2008 and found it no richer in nutrients, while the Ornish program leaves coconut out altogether2829.

Where they differ

The doctors part ways on what saturated fat stands for, what should take its place, how to read the studies said to clear it, how strict to be with coconut, and how low to go1530101131.

Nutrient or marker for animal foods

McDougall said in 2015 that saturated fat is basically animal fat, and that industry prefers the term because shoppers do not picture meat, poultry, cheese and eggs when they hear it32. Campbell argued in 2018 that total fat moved with animal protein at a correlation of about 94 percent across countries, and on a 2020 panel he called the saturated fat story vastly overdone, citing laboratory work from 1909 on that pointed to animal protein as the driver of blood cholesterol1533. Novick answered in 2020 that, like fiber, saturated fat works both as a marker of a diet and as an active ingredient in its own right30. Rip Esselstyn’s site took the same line in 2024, naming saturated fat rather than animal protein alone as the main driver of high cholesterol34.

What should replace it

Willett reported in 2020 that in his cohorts plant monounsaturated and, even more, polyunsaturated fats went with lower mortality, and he prefers unsaturated oils to highly saturated palm oil1035. Jenkins added in 2026 that first-pressed plant oils bring polyphenols that may help the heart20. Esselstyn rejects that swap: in a talk posted in 2009 he cited angiogram studies in people and long monkey studies where arteries narrowed as much on monounsaturated as on saturated fat, and his FAQ says both kinds damage the artery lining3611. Nelson pointed in 2020 to a 1969 trial of 846 veterans, half on a usual diet of about 40 percent fat and half on plant fats at a similar level, with the same rates of heart attack and sudden death, and sided with Nathan Pritikin that total fat matters more than its type3713. Rogers went further in 2026, arguing that polyunsaturated fats do more than saturated fat to make blood clot38.

The studies said to clear it

In 2008 Ornish disputed Gary Taubes’s reading of a large low-carb trial, noting that saturated fat in its Atkins group went up by under a gram a day, and he pointed to a crossover study where the more saturated fat people ate, the worse their blood flow939. Novick wrote in 2020 that the meta-analyses said to clear it looked at no diet that lowered it enough, and that people often replaced it with trans fat, white flour or sugar840. Willett made the comparison against carbohydrate in his own cohorts and found higher mortality with more saturated fat, an analysis he said Ronald Krauss’s review could not do10. Rogers in 2026 called low-carb claims that saturated fat does not cause atherosclerosis ridiculous41. Nelson defended Keys’s Seven Countries study that year and granted that sugar industry money behind a 1967 Harvard review was a real scandal, while insisting it proves nothing about whether saturated fat is safe4243.

Coconut

Barnard objects to coconut oil in food but not on the skin, and in 2026 said coconut milk is the only plant milk he steers people away from, since it can hurt cholesterol much as dairy does344. Alan Goldhamer said in 2020 that coconut milk is high in saturated fat, belongs with nuts and seeds as a portioned rich food rather than a staple, though it may have a use for someone trying to gain weight45. McDougall saw whole coconuts, like nuts and avocados, as harmless foods that are simply too rich for weight loss46. Esselstyn’s FAQ is stricter and advises against even coconut water, for its saturated fat and sugar47. Novick reported that clients who started using tropical oils saw their blood fats worsen and recover once they stopped48.

How low to go

Novick wrote in 2008 that the American Heart Association’s limit was 7 percent of calories, that he preferred under 5 percent, and that three teaspoons of coconut oil already pass that mark on a 2,000 calorie day31. McDougall thinks in total fat instead: his starch-based diet comes to about 7 percent fat, with little of it saturated12. The Ornish program caps all fat at about 10 percent of calories, while Ornish’s 2014 video still listed nonfat dairy and egg whites among acceptable foods2923. In older advice to McDougall, Burkitt suggested semi-skimmed milk and vegetable margarine in place of butter, a swap the no-oil doctors here would not make4911.

What each expert says

John McDougall

McDougall treats saturated fat as another name for meat, poultry, cheese, dairy and eggs, and said in 2015 that the term serves industry by hiding the food behind it32. In 2016 he faulted the US guidelines for naming good foods directly but bad ones only as nutrients like saturated fat and cholesterol14. His own starch diet, he said, is about 7 percent fat and very low in saturated fat12.

Caldwell Esselstyn

Esselstyn said in a 2018 webinar that he learned early to see saturated fat as the culprit in coronary disease, which is why he keeps heart patients off nuts5. He does not accept unsaturated oils as the alternative, citing angiogram and monkey studies in a talk posted in 200936. His FAQ, read in 2026, holds that saturated and unsaturated fat alike injure the arteries, and it advises against coconut water1147.

T. Colin Campbell

Campbell recalled in 2018 how Keys tied fat, mostly the saturated fat of animal foods, to heart disease, and said country data showed no link for plant fat but a steady one for animal fat505152. In his account total fat rose and fell with animal protein so closely that the fat signal mostly reflected animal foods, a point he felt never reached the public1553. In a 2003 talk he rated animal protein as more cholesterol-raising than saturated fat54.

Neal Barnard

Barnard cited Chicago research in 2014 linking about 25 grams of saturated fat a day, against about 13, to more than triple the Alzheimer’s risk22. He calls dairy the leading source and says most of the fat in cheese is saturated2. In 2017 he described a six month Duke study of an Atkins diet in which LDL rose despite weight loss55.

Dean Ornish

In his 2008 reply to TierneyLab readers Ornish argued that a large low-carb trial changed saturated fat intake by too little to clear it, and pointed to a study where blood flow worsened as intake went up939. His Spectrum pages, archived in 2011, rank bad fats as harder to offset with healthy foods than refined carbs56. His program leaves out oils, avocados, olives and coconut29.

Alan Goldhamer

Asked in 2020 whether his center allows light coconut milk, Goldhamer called it high in saturated fat and something to portion like nuts, not a staple45. In 2026, asked why new US advice urged less saturated fat while promoting more meat and full-fat dairy, he said official guidance rarely adds up because it follows politics and special interests57.

Peter Rogers

In a 2026 lecture on fats Rogers said Keys had shown saturated fat to be strongly atherogenic, confirmed from the Seven Countries Study to MRFIT6. Another 2026 lecture blamed saturated fat for dementia, though he also faults other fats and called polyunsaturated fats worse for clotting2438. He calls low-carb denials of the link ridiculous41.

Jeff Novick

Novick wrote in 2020 that his view had not changed despite the meta-analyses, which in his reading never tested a diet truly low in saturated fat8. He sees saturated fat as both a marker of animal foods and harmful on its own, and has watched clients’ blood fats climb after adding tropical oils3048. In 2008 he called coconut oil a cleverly marketed junk food and set his own goal below 5 percent of calories2831.

Jeff Nelson

Nelson argued in 2020 that plant fats are no cure, pointing to a 1969 study of 846 veterans where moving to plant fat did not lower heart attack rates37. In 2026 he defended Keys and said the sugar industry’s hidden funding of a Harvard review does not make saturated fat harmless4243.

Rip Esselstyn

Rip Esselstyn’s PLANTSTRONG site, which also sells his food line, argued in 2024 that saturated fat is the main driver of high cholesterol and that coconut oil, palm oil and processed vegan foods can supply plenty of it344. The same site puts coconut oil at 92 percent saturated fat, and in a talk posted in 2018 he named cheese as probably the largest source2617.

Rip on saturated fat: the full summary on Rip Esselstyn's page

Walter Willett

On a 2020 panel Willett said feeding trials prove saturated fat raises LDL, and that in his cohorts it went with higher mortality than carbohydrate while plant unsaturated fats went with lower710. Dairy and meat supplied most of that saturated fat, he said, which gives one more reason to keep both low1.

Walter on saturated fat: the full summary on Walter Willett's page

David Jenkins

Jenkins said in 2026 that lard and beef tallow clearly raise cholesterol, unlike less processed plant oils20. He added that soy lowers LDL partly by pushing out foods that bring saturated fat and cholesterol58.

Hana Kahleova

In 2025 Kahleova described a randomized trial lasting three weeks, with 32 young adults, where swapping saturated fat for monounsaturated fat from plants made people less angry and lifted their mood59.

Hana on saturated fat: the full summary on Hana Kahleova's page

Denis Burkitt

Burkitt told McDougall that Western diets drew about 45 percent of their energy from fat, most of it saturated and from animals, and he ranked eating less of it among his first priorities49. He blamed farmed meat in particular, which he said carries far more fat than wild game21.

What we do in practice

A diet built on whole starches, vegetables and fruit is low in saturated fat without counting, as McDougall and Esselstyn describe1211. On the plant side, the sources these doctors flag are coconut oil, palm oil and coconut milk345. If your cholesterol is high or you take medicine for it, your targets and any change in treatment are for your own doctor to decide. This page is education, not medical advice.
